MATH DEPARTMENT FORMS

Students in the traditional degree option should submit their Interdisciplinary Application of Mathematics Course Plan by the end of sophomore year.

Discuss your plan with your academic advisor, then submit to the form linked below. Plans must be approved by your advisor and a Traditional Degree Option Chair. The department will email you about final plan approval and documentation in DARS.

Students in the Applied Computational Mathematics (ACM) degree option submit their Interdisciplinary Application of Mathematics Course Plan by the end of sophomore year.

CHANGE OF MAJOR/MINOR/DEGREE OPTION

Math majors can change their math degree option anytime.

  • Complete the College of Science Change of Second Major / Minor / Option Form from the College of Science Forms Page.
  • In the form question that says I would like to ... choose Drop my current option and add the following option.

Students can change their primary major to mathematics during Virginia Tech's designated change of major periods. The next change of major period is November 29, 2024 - January 3, 2025. Note: Students in their first semester at VT must wait until final grades post before requesting to change their primary major to math.

  1. Schedule an Explore Mathematics advising appointment with Jessica Hurdus to discuss your plans for changing your major to math. This can be done anytime.
  2. Apply through Hokie SPA >> Change of Major Application (Undergraduate) during a VT designated change of major period. Click here for step-by-step instructions.

Continuing students can add math as a second major anytime. Students in their first semester at VT must wait until final grades post before requesting to add math as a second major.

  1. Schedule an Explore Mathematics advising Appointment with Jessica Hurdus to discuss your plans for changing your major to math. This can be done anytime.
  2. Complete the College of Science Change of Second Major / Minor / Option Form from the College of Science Forms Page.    
  • In the form question that says I would like to ... choose Add a second major.

Students can add or drop a math minor anytime. No appointment is necessary.

  • Complete the College of Science Change of Second Major / Minor / Option Form from the College of Science Forms Page.    
  • In the form question that says I would like to ... choose Add a minor or Drop a minor as appropriate.
  • In the next form question, select Minor: Mathematics (MATH) 

COLLEGE OF SCIENCE FORMS

Details about select forms found on the College of Science Forms Page are included below. Most of the forms are for undergraduate students whose primary major is in the College of Science. Please direct any further questions to your advisor (math majors) or mathadvising@vt.edu (non-majors).

If you are taking classes at a Virginia Community College System (VCCS), you do not need to complete an Authorization to Take Courses Elsewhere (US) Form. VCCS courses have been pre-approved and are listed in the VCCS Course Equivalents transfer guide: https://transferguide.registrar.vt.edu.

If your primary major is in the College of Science and if you are taking classes at a college or university in the United States that is not part of the Virginia Community College System (VCCS), follow the steps below.

To submit the Authorization to Take Courses Elsewhere (US) Form:

  1. Discuss your course plans with your academic advisor.
  2. Download the Authorization to Take Courses Elsewhere (US) Form from the College of Science Forms Page.
  3. Complete all sections of the form, including the student signature field. You will need to run a DARS report to fill in the boxes on page 2.
  4. Submit the completed form to your advisor's document portal linked below. (Never email a form that contains your full name and ID number.) Your form will be reviewed and you will be notified of any necessary changes via email. If you have not received an email within one business week of form submission, email your advisor. Once any necessary updates are made (by you), the form will be signed and submitted to the College of Science on your behalf.

Allow up to six weeks for processing. You will receive an email from the College of Science indicating whether your transfer credit has been approved.

This directions below are for students whose primary major is in the College of Science. Directions for completing the Authorization to Take Courses Abroad Form can be found on the College of Science Forms Page. Step-by-step directions are listed on the top of the form itself. Follow these directions carefully. 

  1. Download the Authorization to Take Courses Abroad Form from the College of Science Forms Page
  2. Meet with an advisor in the Global Education Office and obtain their signature on the form as indicated.
  3. Discuss your plans with your advisor.
  4. Submit the completed Authorization to Take Courses Abroad Form and submit a syllabus for each class abroad listed in the form to your advisor's document portal linked below. (Never email a form that contains your full name and ID number.) Your form will be reviewed and you will be notified of any necessary changes via email. If you have not received an email within one business week of form submission, email your advisor. Once any necessary updates are made (by you), the form will be signed and a bundle of form+syllabi will be submitted to the College of Science on your behalf.

Allow two to three months for processing. You will receive an email from the College of Science indicating whether your transfer credit has been approved.

To apply for a course withdrawal, complete the Course Withdrawal Form found on the College of Science Forms Page. Some notes about course withdrawal are below.

  • Requesting a course withdrawal (W grade policy) will denote a final grade of "W" on your transcript for each course in which a Course Withdrawal is applied. No credit is earned for a course with a W grade, and a W grade does not factor into GPA. Withdrawing from a course will not change full-time student status, as the course(s) withdrawn from will remain on your transcript (though no credit is earned). 
  • Students are permitted to withdraw from up to three courses during their time at Virginia Tech. 
  • The deadline to apply for a course withdrawal in a particular semester is the last day of classes of that semester. 
  • An advisor's signature is not required on the form, but it is highly recommended to discuss a course withdrawal decision with your advisor.
